How to Copy a Form with Custom Form Builder
You have the option of creating a new form or copying one of the numerous forms provided by CDLSuite. On your profile home page, choose Custom Forms.

*Pressing Preview allows you to see your form. Pressing Mobile Preview allows you to see the streamlined mobile version of the form.
Copy A Form
To copy a form, choose the form that you want from the list and click on Copy Form. The page will refresh and add your form, adding the word “copy”. At this point, you can edit the form, delete it, or use it as is.
“Active/Inactive” gives the option to make your form active when you are ready to use it. The Staff option should be chosen when your form is to be used internally only. Staff can complete and make changes to the form using information from the candidate’s application.
“Auto Send” refers to how the form will be delivered to applicants. Choose Always to have your form automatically delivered to every applicant. Choose Conditional to deliver your form to applicants only after they have answered your custom questions. (Requires you to create a custom question.) Turn Off this section if you don’t want you use this option.
“Date”
The Date reflects when the form was created/copied or edited.
"Form Permissions"
This option allows you to give permissions to users that you established on your profile home page.

Editing Your Copied Form
Choose the form that you copied and wish to customize. Click on the blue pencil in the Options column to go to the form. Scrolling through the form will highlight individual fields which can be edited or deleted.
General Fields
Add new fields by dragging fields from the control panel on the left or by simply clicking on the desired field element, which will add it to the last field position on the field canvas. To edit, simply click on the box.
Application Fields
Application fields are those fields that will prepopulate with the applicant’s information when signing into his/her account.
Check the "Add Upload File button" box to allow applicants to upload any file that you request. Checking the “Add Your Company Logo” box will automatically add the logo you uploaded to your profile.
Create/Preview
Toggling these buttons allows you to move from Creation to Preview and back.
Save/Cancel
Save your form when ready or cancel without saving anything.
